This is the BHOA Membership Card Request form. Please remember that residents must have a membership card to use the facilities of the Community Center, and that each family must renew their request each year. Each Belair household will also receive visitor cards for use with guests. Renters in Belair must have a letter from the owner on file with the Belair Homeowners Association office granting usage of their membership privileges. The prior year cards will only be “good” until the end of February. ID cards are required for all activities and events at the Community Center. Please complete and return this form by mailing it to the above address, handing it to the Manager at the Community Center, or submitting this form online.
After verification of your status as a “member in good standing” with the Treasurer and the Covenant Committee, your form will be processed. This Membership Card Request Form must be fully completed by the homeowners so that we can gather the most accurate information from which to issue your Membership Cards. Each child as well as the adults will need his/her own membership card. The Membership Cards will allow access to the Community Center, tennis courts and the pool during the times of operation.
All membership cards are photo IDs. Most will not be required to re-take their picture, but those without a photo on file with BHOA will need to have one taken before your form can be processed. BHOA also reserves the right to request any member re-take their photo. We will resume taking photos after January 23rd. Check the website or the Community Center door for office hours.
SPECIAL NOTE --- An incomplete form will delay your request.
Please note that our card process takes time. There can be no “Same Day Service” with regard to the request for membership cards. Members will not be able to use the facility until their cards can be completed. Please do not wait until the day you wish to use the facility to request cards.
Those unsure of their lot number can check their previous card or they may contact the Community Center online here or call 985-781-1002.
Once the cards are ready, they must be signed out from the Community Center by the homeowner.
Please complete all the requested information below including the name of each permanent resident in your household along with their birthday. We ask for the birthday in order to determine the appropriate age-based card to issue for each individual. We will attempt to call you when your cards are ready.
